A complete application should include the following:

1. Complete form ISBE 73-03D School Service Personnel or Administrative.

2. Complete form ISBE 73-63 Application for Issuance of Provisional.

3. Complete form ISBE 73-05 State Approved Program Verification. This must be completed by the applicant and must be signed and sealed by the university. This form needs to be sent directly to the Illinois State Board of Education, Division of Certification, 100 North First Street, Springfield, IL 62777-0001 from the university.

4. Complete form ISBE 73-99 Confirmation of State Certification Tests Taken. Illinois will honor basic skills and/or content area tests taken in another state where these tests were required for certification. The applicant should fill out Part 1 and submit the form to the State Certification Authority of the state where the test was taken for completion. 5. Official transcripts from all schools attended in original sealed envelopes.

6. Photocopy (front and back) of out-of-state certificate.
7. Application fee in the amount of $30.
8. Letters of teaching or administrative experience should include dates of employment, subjects/grade levels taught or positions assigned:
   • General Supervisory and General Administrative "requires verification of two years full-time teaching or school service personnel experience.
   • Superintendent "requires verification of two years of administrative experience and possession of the general supervisory or general administrative of a comparable out-of-state certificate.
   • Director of Special Education requires verification of two years of full time teacher or school service personnel in a field other than school nurse.
