A complete application should include the following:
1. Complete form ISBE 73-03C Application for Teaching Certificate.

2. Complete form ISBE 73-10 Certificate Registration Form.
3. Complete form ISBE 73-05 State Approved Program Verification.
This must be completed by the applicant and should be signed and sealed by the university. The university needs to send this form directly to the Illinois State Board of Education at:

4. Complete form ISBE 73-99 Confirmation of State Certification Tests Taken.
Illinois may honor the basic skills and/or content area test taken in another state if these tests were required for certification. The applicant should fill out Part 1 and submit the form to the State Certification Authority of the state where the test was taken for completion. 5. Official transcripts from all schools attended in original sealed envelopes. .
6. Photocopy (front and back) of out-of-state certificate
7. Complete form ISBE 73-63 Application for Issuance of Provisional Certificate.
8. $30.00 money order or check made payable to ROE #40. Please call for additional registration fees.
Registration fees are as follows:

- Standard, Provisional Vocational certificate(s): $25.00
- Initial certificate(s): $20.00
- Transitional Bilingual certificate(s): $30.00
**If you have four years or more of teaching experience, please apply for a standard certificate. You will need to include letters of four years' teaching experience on official school letterhead.**
