A complete application should include the following:
  
1. Complete ISBE Form 73-52 Application for Endorsement of Issued Certificate
2. $30.00 money order or check made payable to the Regional Office of Education #40.
3. Official transcripts in original sealed envelopes not previously filed with the Illinois State Board of Education.
  
Please submit your complete application and fees to the Regional Office of Education #40.
  
Once you get your application evaluated, the Illinois State Board of Education will send deficiency letters to explain to you if you are missing any requirements to get the endorsement(s) you have applied for or they will issue you a certificate with the endorsement(s) listed.
Once you meet the requirements, please send in proof (i.e. official transcripts, course descriptions, letters) to our office with a copy of your deficiency letter. Your application is good for the evaluation time deadline stated in your letter. You will need to fulfill the deficiencies before the deadline. If you do not meet the requirements in time, you will need to reapply for the endorsement(s).
